Deborah Zahn is the Director of External Affairs and the Interim Director of Performance Improvement for the Primary Care Development Corporation (PCDC). In this position, Ms. Zahn uses her extensive experience in both the policy and program arenas to support primary care centers and improve access to high quality care for underserved communities. Before moving to New York, Ms. Zahn was the Director of three of the cornerstone projects for the uninsured and underserved in Alameda County, CA including the Robert Wood Johnson Foundation-funded Communities In Charge Project, the W.K. Kellogg Foundation-funded Community Voices for Immigrant Health project, and The California Endowment and California HealthCare Foundation-funded Frequent Users of Health Services Initiative. Ms. Zahn also was the Director of the Alameda Health Consortium’s American Legacy Foundation-funded Tobacco-Use Intervention Project. This project implemented and expanded cessation services at local community health centers. Previously, Ms. Zahn was a Program Manager at Kaiser Permanente where she developed and implemented the highly successful self-care program and other health programs across 32 medical centers in Northern California. |
